The Wraftful, super-powered Omnipower Malignant Heart can only be acquired in World Tier 4 (Torment) difficulty, while the reset can be farmed in any difficulty level. You can get Malignant Hearts in Malignant Tunnels or out in the world of Sanctuary as you slay the followers of Lilith, who have been “corrupted.”
