One such piece of side content involves the Mission Item Damaged Music Box. How To Get The Damaged Music Box In Honkai Star Rail

In order to acquire the Damaged Music Box, players will need first to acquire the Dirty Ribbon item. Players should kill the Warp Trotter in Rivet Town, and after acquiring this item, visit the window of the bridge and interact with the character there (initially referred to as "Weak Female Voice").

Give her the item, and she will reward you with the Damaged Music Box. The Damaged Music Box will have the following in-game description: 

"A small, handcrafted music box. Though kept under good care for many years, it has fallen into a state of disrepair. The melody produced by the wooden box was somewhat unsettling, nonetheless, it was cherished by the two girls, who relied on each other like sisters.
“Welcome… home…”

How To Use The Damaged Music Box In Honkai Star Rail

Once the Damaged Music Box is in your possession, return to Rivet Town and talk with the Little Sister. Hand her the Damaged Music Box, and she will reward you with a Praise of High Morals and the Outworlder Achievement. 

This quest is often referred to as the "Honkai Star Rail Bloodborne Easter Egg" because of a similar quest in Bloodborne that involves a music box and speaking with a girl through a window.
