There are four Malignant Hearts that are specific to the Barbarian class in Diablo 4 Season 1. Remember, you need to create a Seasonal Character to get these drops. This is a post-campaign feature with a questline starting with the new NPC, Cormond, so these will not drop during the story.

The Ignoring Pain Wrathful Malignant Heart only starts dropping at World Tier 4 difficulty, while the rest can drop at any difficulty level.
It is important to note that the stats for Barbarian-specific Malignant Hearts, as shared by Blizzard, are for drops with level 20 characters. When the item drops, these stats grow in power the higher your character level at the time.
Please note: The Barbarians can also use the 12 general Malignant Hearts for all classes.
