There are a lot of affixes in Diablo 4, and sometimes, their effects can get a bit confusing. If you are wondering what the Healthy status effect on enemies means or what "while healthy" means on a skill/affix in Diablo 4, we've got you covered.

This article explains Diablo 4's Healthy status effect in detail for both enemies and the player character and how it translates into general gameplay, so let's get started.

How Healthy Status Effect Works in Diablo 4

The Healthy status effect, as you might have guessed, is the exact opposite of Diablo 4's Injured effect. You will spot the affixes with a Healthy effect on items and some skills, Glyphs, and more. For example, you might see a skill improvement such as the Necromancer's Hemorrhage note "gains an additional 20% attack speed while Healthy."

Another example is how an item affix might provide a percentage increase to damage to "Healthy Enemies." Check out the quick and straightforward explanation of what Healthy means in Diablo 4 below:

Healthy means above the enemies, or your character is above 80% health.

There's also one additional caveat. Some affixes say "while Healthy." For example, you do increased damage while Healthy...This means the player needs to stay above 80% Health to get the benefit. If the player drops below 80% health even for a second, the clock on this effect is reset.

Regarding gameplay, if you decide to stack affixes with the Healthy affix, you will need to frontload your damage, allowing you to do huge damage numbers on those first hits. To remain healthy and keep the "While Healthy" benefit, we suggest using skills that trigger Fortify or Barrier to further protect your health bar from depleting.
